Welcome to Radji Barrett Jiu-Jitsu Academy.

Our goal is to provide a safe, respectful, and welcoming environment where students can learn Brazilian Jiu-Jitsu and martial arts, build confidence and discipline, stay active, and become part of a positive community.

Brazilian Jiu-Jitsu and martial arts are physical contact activities and involve inherent risks. Please review the following terms carefully before participating or permitting a minor child to participate.

SECTION 01

Brazilian Jiu-Jitsu and martial arts training involve close physical contact with instructors and training partners. Activities may include drilling, grappling, positional training, live rolling or sparring, takedowns, throws, submissions, joint locks, chokes, self-defense techniques, striking activities, strength and conditioning exercises, open mats, seminars, competitions, and other physical activities.

While Radji Barrett Jiu-Jitsu Academy works to provide a safe training environment, no contact sport or physical activity can be completely free from risk.

I understand that participation may result in injuries including, but not limited to, bruises, cuts, strains, sprains, joint or ligament injuries, fractures, dislocations, dental injuries, concussions, head or neck injuries, illness, communicable infections, serious bodily injury and, in rare circumstances, paralysis or death.

These injuries may result from the participant's own actions, the actions of another participant or training partner, physical contact inherent in training, instruction or supervision, equipment, mats, facilities, or other circumstances associated with martial arts participation.

By signing this agreement, I knowingly and voluntarily accept and assume these risks for myself or, if signing for a minor, on behalf of the minor participant.

SECTION 02

All students are expected to contribute to a safe and respectful training environment.

Participants agree to follow reasonable instructions from coaches and academy staff; respect training partners and fellow members; immediately respect physical or verbal taps and submissions; train with appropriate control; avoid intentionally reckless or dangerous behavior; maintain appropriate personal hygiene and clean training clothing; disclose injuries or physical limitations that could affect safe participation; and refrain from training when knowingly experiencing a contagious illness or skin condition that may place others at risk.

Participants are also expected to treat the academy, its staff, members, visitors, facilities, and property respectfully.

Radji Barrett Jiu-Jitsu Academy reserves the right to restrict, suspend, or terminate participation when conduct creates a safety concern or violates academy rules or policies.

SECTION 03

I understand that it is my responsibility, or the responsibility of the parent or legal guardian signing for a minor, to determine whether participation is appropriate based on the participant's health and physical condition.

Relevant injuries, allergies, medical conditions, medications, or physical limitations that could reasonably affect safe participation should be communicated to academy staff. If there is uncertainty regarding a participant's ability to safely participate, an appropriate healthcare professional should be consulted.

Radji Barrett Jiu-Jitsu Academy, its instructors, coaches, staff, and representatives do not provide medical advice.

SECTION 04

If an injury or medical emergency occurs during an academy activity, I authorize Radji Barrett Jiu-Jitsu Academy and its staff to contact emergency medical services and/or the participant's emergency contact when reasonably necessary.

If I am signing for a minor and I cannot be reached in a timely manner, I authorize reasonable emergency care to be sought for the minor participant.

I understand that academy staff are not medical professionals and that I am responsible for medical expenses incurred on behalf of myself or my child.

SECTION 05

PLEASE READ THIS SECTION CAREFULLY. IT AFFECTS LEGAL RIGHTS.

In consideration for being permitted to participate in activities offered by Radji Barrett Jiu-Jitsu Academy, I voluntarily assume the inherent and other risks associated with Brazilian Jiu-Jitsu, martial arts, striking, fitness activities, open mats, seminars, competitions, events, and use of the academy's premises, mats, facilities, and equipment.

To the fullest extent permitted by applicable New Jersey law, I release and waive claims against Radji Barrett Jiu-Jitsu Academy and its owners, instructors, coaches, employees, agents, representatives, and other persons acting on its behalf arising from the ordinary risks associated with participation and, to the extent legally enforceable, ordinary negligence associated with participation in academy activities or use of academy premises, facilities, or equipment.

I understand that by signing this agreement I may be giving up important legal rights.

Nothing in this agreement is intended to waive, release, or limit liability that cannot legally be waived, released, or limited under applicable law.

SECTION 06

Participants are responsible for their personal belongings while at the academy or participating in academy activities.

To the extent permitted by law, Radji Barrett Jiu-Jitsu Academy is not responsible for personal property that is lost, stolen, misplaced, or damaged.

SECTION 07

If I am signing this agreement on behalf of a participant under 18 years of age, I represent that I am the participant's parent or legal guardian, or otherwise have legal authority to provide consent for the minor's participation.

I understand the nature of the Brazilian Jiu-Jitsu and martial arts activities described in this agreement, acknowledge the associated risks, and voluntarily give permission for the minor participant to participate.

By signing this agreement, I agree to its terms both in my capacity as the minor participant's parent or legal guardian and on behalf of the minor participant to the extent permitted by applicable law.

SECTION 08

Radji Barrett Jiu-Jitsu Academy may occasionally photograph or record classes, promotions, seminars, competitions, team events, and other academy activities.

Photos and videos may be used on the academy's website, social media, digital advertising, email, printed materials, and other academy-related promotional or educational content.

Please select one:

YES — I authorize Radji Barrett Jiu-Jitsu Academy to photograph and/or record me or my minor child and permit the academy to use those photographs or recordings for academy-related promotional, educational, website, social media, advertising, and marketing purposes without compensation.

NO — I do not authorize Radji Barrett Jiu-Jitsu Academy to use identifiable photographs or video of me or my minor child for promotional or marketing purposes.

Choosing NO will not affect membership or participation in academy classes.

SECTION 09

By signing below, I confirm that I have read and understand this entire agreement.

I understand that Brazilian Jiu-Jitsu and martial arts are physical contact activities involving inherent and other risks, and I voluntarily choose to participate or permit my minor child to participate with knowledge of those risks.

I understand the academy's safety and conduct expectations, health and medical responsibilities, and the Assumption of Risk and Release and Waiver of Liability contained in this agreement.

If I am signing on behalf of a minor, I confirm that I am the participant's parent or legal guardian, or otherwise have legal authority to provide this consent.

My signature below applies to and signifies my agreement to this entire document.
